organized as a limited liability company under the laws of the state of Nevada and has been in business since 2024.
WELLth Advisory Services, LLC (someNmes referred to herein as “we,” “us” or “WELLth Advisory Services”) is owned
by SRP Holdings Group, LLC, a Delaware corporaNon. The WELLth Advisory Services main office is located at 25434
Prairiewood Lane, Shorewood, Illinois 60404. The WELLth Advisory Services Senior Management Team consists of
Jeff Cullen, Chief ExecuNve Officer; Deane Mayerhofer, Chief OperaNng Officer; and Sarah Hughes, Chief Compliance
Officer.
WELLth Advisory Services recognizes that the advisor’s role should extend beyond investment guidance and ohen
requires a variety of coordinated financial service strategies to create a roadmap to achieving financial goals for their
clients. We support this effort by providing our Investment Advisory RepresentaNves (“IARs”) with the tools and
resources to meet client needs and objecNves.
Re#rement Plan Services
We approach client service with a collaboraNve style delivered by a dedicated plan consultant team in our regional
offices. This team is further supported by specialists who are specifically focused in the areas of ERISA Compliance,
Investment Research, Provider Analysis, Fee Benchmarking, Employee EducaNon, and ExecuNve Benefits. We wake

WELLth Advisory Services, through its affiliated IARs, provides consulNng and advisory services to both ERISA and
Non-ERISA employer sponsored reNrement plans, including, but not limited to, 401(k), 457(b), 457(f), 403(b), Simple
IRA, SEP IRA, nonqualified, deferred compensaNon, pension and profit-sharing plans (collecNvely, “Plans” or
individually, “Plan”) on both a one-Nme and/or ongoing basis.
WELLth Advisory Services offers a suite of detailed engagement agreements which are customized for each client
relaNonship and executed by the Plan’s designated fiduciary upon conclusion of a careful review, which, at Nmes,
includes the client’s independent legal counsel. Through its agreements, WELLth Advisory Services is engaged to
provide investment advisory services on either a “nondiscreNonary” basis (serving as a “fiduciary” as defined by
SecNon 3(21)(A)(ii) of the Employee ReNrement Income Security Act of 1974 (“ERISA”)); or on a “discreNonary” basis
and thus will serve as an “Investment Manager” as defined by SecNon 3(38) of ERISA. Certain other addiNonal
services available from WELLth Advisory Services would be considered non-fiduciary by definiNon and funcNon and
are explicitly detailed within the Plan’s executed agreement.
For non-discreNonary services, WELLth Advisory Services and its IARs will act in a solely advisory capacity and will
not have or exercise any discreNonary authority or control relaNve to the management or investment of the assets
of the respecNve Plan.
For discreNonary services, WELLth Advisory Services and its IARs will be designated as the Investment Manager to
the Plan and assume responsibility for the investment selecNon and asset management for the Plan’s master menu
made available to the Plan parNcipants from which to choose. In all cases, WELLth Advisory Services will not serve as
the “named fiduciary” of the Plan.
Our agreements offer our clients the opportunity to select one or more of the following services in various
engagement categories:
The Plan-level Fiduciary Services that are provided under the Agreement include:

o Ongoing Investment Monitoring.
o Qualified Default Investment AlternaNve.
o DiscreNonary Model Pormolios.
The Plan-level Non-Fiduciary Services may include:
• Service Provider Liaison.
• EducaNon Services to Plan Comminee.
• Plan Search Support/Vendor Analysis.
• Benchmarking Services.
• Assistance IdenNfying Plan Fees.
• Plan Design ConsulNng.
• Plan Review.
ParNcipant-level Services may include:
• ParNcipant Enrollment.
• ParNcipant EducaNon.
• Comprehensive Wellness Program.
Based on the needs of the client, the agreement will specify the services on a client-by-client basis.
Wealth Management Services
WELLth Advisory Services offers investment advisory, pormolio management, and financial planning services to
individuals and families. Our investment recommendaNons primarily include mutual funds, exchange-traded funds,
and separate account managers invesNng in exchange-listed equity securiNes. The advice provided by WELLth
Advisory Services is tailored to the unique objecNves of each client. We work with clients to formulate an investment
strategy aher discussing risk tolerance, Nme horizon, and projected future liquidity needs, current holdings, tax
consideraNons, personal market views and other factors. This strategy provides guidance to formulate suitable
investment and financial recommendaNons. We meet with clients as needed to review pormolio performance,
discuss current issues, and reassess goals and investment plans. Client input, involvement and decision-making are
criNcal to the planning process and implementaNon of investment decisions.
WELLth Advisory Services also offers financial plans. These financial planning services are based on the client’s
financial situaNon at the Nme the financial informaNon is disclosed by the client to WELLth Advisory Services. Clients
are advised that certain assumpNons may be made with respect to interest and inflaNon rates and the use of past
trends and performance of the market and economy. Because clients’ financial situaNons, goals, objecNves, or needs
change, clients are encouraged to noNfy us promptly if they wish to update their financial plan.
WELLth Advisory Services also offers discreNonary management services to our individual clients. Regardless of
whether our authority is discreNonary or non-discreNonary, clients may impose reasonable restricNons on invesNng
in certain securiNes or types of securiNes.
The asset management program sponsors uNlized by WELLth Advisory Services include, but are not limited to the
LPL Financial, LLC (“LPL”) sponsored advisory programs listed below:
Manager Access Select Program
Manager Access Select offers clients the ability to parNcipate in the Separately Managed Account Plamorm (the “SMA
Plamorm”) or the Model Pormolio Plamorm (the “MP Plamorm”). In the SMA Plamorm, WELLth Advisory Services will
assist client in idenNfying a third-party pormolio manager (SMA Pormolio Manager) from a list of SMA Pormolio
Managers made available by LPL, and the SMA Pormolio Manager manages client’s assets on a discreNonary basis.
WELLth Advisory Services will provide iniNal and ongoing assistance regarding the SMA Pormolio Manager selecNon
process. In the MP Plamorm, clients authorize LPL to direct the investment and reinvestment of the assets in their
accounts, in accordance with the selected model pormolio provided by LPL’s Research Department or a third-party
investment advisor.
A minimum account value of $50,000 is required for Manager Access Select, however, in certain instances,
the
minimum account size may be lower or higher.
Op3mum Market Por6olios Program (OMP)
OMP offers clients the ability to parNcipate in a professionally managed asset allocaNon program using OpNmum
Funds shares. Under OMP, the client will authorize LPL on a discreNonary basis to purchase and sell OpNmum Funds
pursuant to investment objecNves chosen by the client. WELLth Advisory Services will assist the client in determining
the suitability of OMP for the client and assist the client in seqng an appropriate investment objecNve. WELLth
Advisory Services will have discreNon to select a mutual fund asset allocaNon pormolio designed by LPL consistent
with the client’s investment objecNve. LPL will have discreNon to purchase and sell OpNmum Funds pursuant to the
pormolio selected for the client. LPL will also have authority to rebalance the account.
A minimum account value of $10,000 is required for OMP. In certain instances, LPL will permit a lower minimum
account size. Account fees are payable quarterly in advance.
Personal Wealth Por6olios Program (PWP)
PWP offers clients an asset management account using asset allocaNon model pormolios designed by LPL. WELLth
Advisory Services will have discreNon for selecNng the asset allocaNon model pormolio based on client’s investment
objecNve. WELLth Advisory Services will also have discreNon for selecNng third-party money managers (PWP
Advisors), mutual funds and ETFs within each asset class of the model pormolio. LPL will act as the overlay pormolio
manager on all PWP accounts and will be authorized to purchase and sell on a discreNonary basis mutual funds, ETFs
and equity and fixed income securiNes.
A minimum account value of $250,000 is required for PWP. In certain instances, LPL will permit a lower minimum
account size. Account fees are payable quarterly in advance.
Model Wealth Por6olios Program (MWP)
MWP offers clients a professionally managed mutual fund asset allocaNon program. WELLth Advisory Services will
obtain the necessary financial data from the client, assist the client in determining the suitability of the MWP program
and assist the client in seqng an appropriate investment objecNve. WELLth Advisory Services will iniNate the steps
necessary to open an MWP account and have discreNon to select a model pormolio designed by LPL’s Research
Department consistent with the client’s stated investment objecNve. LPL’s Research Department, a third-party
pormolio strategist and/or WELLth Advisory Services, through its IAR, may act as a pormolio strategist responsible for
selecNng the mutual funds or ETFs within a model pormolio and for making changes to the mutual funds or ETFs
selected.
The client will authorize LPL to act on a discreNonary basis to purchase and sell mutual funds and ETFs and to liquidate
previously purchased securiNes. The client will also authorize LPL to effect rebalancing for MWP accounts. MWP
requires a minimum asset value for a program account to be managed. The minimums vary depending on the
pormolio(s) selected and the account’s allocaNon amongst pormolios. The lowest minimum for a pormolio is $25,000.
In certain instances, a lower minimum for a pormolio is permined. Account fees are payable quarterly in advance.
Small Market Solu3on (SMS) Program
Under SMS, LPL Research (a team of investment professionals within LPL) creates and maintains a series of different
investment menus (“Investment Menus”) consisNng of a mix of different asset classes and investment vehicles
(“investment opNons”) for clients that sponsor and maintain parNcipant-directed defined contribuNon plans (“Plan
Sponsors”). The Plan Sponsor is responsible for selecNng the Investment Menu that it believes is appropriate based
on the demographics and other characterisNcs of the Plan and its parNcipants. LPL Research is responsible for the
selecNon and monitoring of the investment opNons made available through Investment Menus. The investment
opNons that are offered through SMS are limited to the specific investments available through the record keeper that
the Plan Sponsor selects. The Plan Sponsor may only select an Investment Menu in its enNrety and does not have
the opNon to remove or subsNtute an investment opNon.
In addiNon to the services described above, Plan Sponsor may also select from a number of consulNng services
available under SMS that are provided by WELLth Advisory Services. These consulNng services may include, but are
not limited to general educaNon, and support regarding the Plan and the investment opNons selected by Plan
Sponsor; assistance regarding the selecNon of, and ongoing relaNonship management for, record keepers and other
third-party vendors; Plan parNcipant enrollment support; and parNcipant-level educaNon regarding investment in
the Plan. These consulNng services do not include any individualized investment advice to the Plan Sponsor or Plan
parNcipants with respect to Plan assets. Account fees are payable in arrears based on the frequency agreed to
between the client and WELLth Advisory Services.
Guided Wealth Por6olios (GWP)
GWP offers clients the ability to parNcipate in a centrally managed, algorithm-based investment program, which is
made available to users and clients through a web-based, interacNve account management portal (“Investor Portal”).
Investment recommendaNons to buy and sell exchange-traded funds and open-end mutual funds are generated
through proprietary, automated, computer algorithms (collecNvely, the “Algorithm”) of FutureAdvisor, Inc.
(“FutureAdvisor”), based upon model pormolios constructed by LPL and selected for the account as described below
(such model pormolio selected for the account, the “Model Pormolio”). CommunicaNons concerning GWP are
intended to occur primarily through electronic means (including but not limited to, through email communicaNons
or through the Investor Portal), although WELLth Advisory Services will be available to discuss investment strategies,
objecNves, or the account in general in person or via telephone.
A preview of the Program (the “EducaNonal Tool”) is provided for a period of up to forty-five (45) days to help users
determine whether they would like to become advisory clients and receive ongoing financial advice from LPL,
FutureAdvisor and WELLth Advisory Services by enrolling in the advisory service (the “Managed Service”). The
EducaNonal Tool and Managed Service are described in more detail in the GWP Program Brochure. Users of the
EducaNonal Tool are not considered to be advisory clients of LPL, FutureAdvisor or WELLth Advisory Services, do not
enter into an advisory agreement with LPL, FutureAdvisor or WELLth Advisory Services, do not receive ongoing
investment advice or supervisions of their assets, and do not receive any trading services. A minimum account value
of $5,000 is required to enroll in the Managed Service.
Poten3al Conflicts of Interest
TransacNons in LPL advisory program accounts are generally made through LPL as the execuNng broker-dealer.
WELLth Advisory Services receives compensaNon as a result of a client’s parNcipaNon in an LPL program. Depending
on, among other things, the size of the account, changes in its value over Nme, the ability to negoNate fees or
commissions, and the number of transacNons, the amount of this compensaNon may be more or less than what
WELLth Advisory Services would receive if the client parNcipated in other programs, whether through LPL or another
sponsor, or paid separately for investment advice, brokerage, and other services.
Assets Under Management and Advisement
As of March 26, 2024, WELLth Advisory Services manages a total of $0M regulatory assets under management, which
includes $0M of discreNonary assets under management, and $0M of non-discreNonary assets under management.
